By submitting your question or story, you agree to the terms outlined below regarding its use on the Workplace Weirdness Podcast (the “Podcast”).

1. By signing below, I, the undersigned, hereby consent to the recording, use, and distribution of my submitted question(s) and/or story (“Listener Contribution”) for the Podcast.
2. I grant Green Banana Consulting LLC and its employees, agents, heirs, successors, assigns, licensees, affiliates, parents, subsidiaries, and legal representatives (together, the “Company”) the irrevocable worldwide rights and permission to use, modify, license, and broadcast my Listener Contribution, in whole or in part, in any form or media associated with the Podcast and/or with any initiatives associated with the Company or the Podcast, including without limitation any promotional materials or derivative works.
3. I acknowledge that any work product created by the Company using my Listener Contribution, in whole or in part, is the sole property of the Company, and I hereby waive any rights therein.
4. I understand that only the content of the story or question I provide will be used, and I acknowledge that no personal identifying information will be shared.
5. I confirm that my Listener Contribution is my original work and that it does not infringe on the rights of third parties, nor does it contain defamatory, unlawful, or otherwise inappropriate material.
6. I agree that the Company is under no obligation to compensate me for my contribution.
7. I release and forever discharge the Company from any and all claims, demands, damages, or liabilities arising from the use of my Listener Contribution, including but not limited to any claims for invasion of privacy, defamation, or infringement of intellectual property rights.
8. This Consent is governed by the laws of the State of Florida, and the courts of the State of Florida shall have exclusive jurisdiction over any disputes arising out of or in connection with this Consent. This Consent may not be changed orally.
